Firdapse (amifampridine) is one of the few FDA-approved treatments specifically for LEMS, and for many patients, it’s a game-changer in restoring muscle strength and improving daily function.

But it’s not always simple. Dosing, timing, and side effects can vary widely from person to person—and finding the right balance takes time.

Whether you’re just starting Firdapse, have been taking it for years, or are considering it, this is a space to share real-life experiences and advice.

If you have taken Firdapse, how long have you been on Firdapse, and what improvements did you notice?

What dosage and schedule work best for you? (How many times per day? Spaced how far apart?)

Have you experienced any side effects like tingling, anxiety, headaches, or GI issues?

Do you take it on an empty stomach, with food, or adjust timing to fit your daily routine?

If you’re living with LEMS and taking Firdapse, your experience can really help others—especially those who are newly diagnosed or just starting treatment. What’s worked (or not worked) for you?

I was diagnosed with LEMS in Feb 2023, I am now 73 years old. I started 100mg of firdapse as soon as it was approved by the FDA. In Feb 2023. I took 80mg of Firdapse daily. My frequency has changed over time. I set my alarm for 6,9 am, noon, 3pm and 6pm. This worked for some time. I took my 6am dose and stayed in bed for about 1 hour before rising, this caused heartburn at times. Every day varied, some good, some not good . About 10 days ago I changed my schedule to 6, 8:30 am and 12 noon, 4 pm and 9pm. On day one of this change I took my 6am and immediately was able to rise and I felt very good! I hope this does last, I have not felt this good since 2023. While this is new, and it has helped me, I do not recommend it for others without first consulting with your doctor. Please share your experiences with Ashley as your experience may be very helpful to others .
